CPD Application Form  (pdf)


It is the goal of Sonography Canada to promote and provide continuing professional development (CPD) opportunities for all Canadian Sonographers. The educational activities that are granted CPD credits facilitate the accumulation of credits for both SONOGRAPHY CANADA and ARDMS purposes.  Sonography Canada will not grant credits for programs, which have already been granted AMA category 1, or SDMS credits.

It is the responsibility of the Program Coordinator to ensure that the completed application documents and fees are submitted to Sonography Canada at least 6 weeks prior to the event.

NOTE: Credits will not be granted retroactively.

Sonography Canada will approve credits for the following type of activities:

Two different types of credit; Sonography Based and Relevant to Practice, will be awarded at rate of 1 per hour or less than 1 hour at 0.25 credits per 15-minute interval.  The type of credits awarded will depend on activity content. CPD activity certificates must state how many of each type of credits were received.

Sonography Based

  • Must directly involve the practice of sonography.
  • Must enrich the member in a direct way by improving sonography knowledge, performance and competency to directly improve patient care.

Relevant to Practice

  • Not sonography based but enhance professional practice.
  • These activities enable the member to expand and improve their knowledge, skills and inter-professional relationships, promoting and enhancing the profession of diagnostic medical sonography and ultimately enhancing patient care.

General Information

  • Application for Sonography Canada CPD credits must be submitted with the required documentation, and the applicable processing fee. Proof of a current and valid chapter/corporate membership (photocopy of membership card) is required to use the members' discounted rate. The application fee is found on the application form provided. A cheque, money order, or credit card payment for the full amount made out to Sonography Canada must accompany the application. This fee is non-refundable.
  • If a CPD activity will be offered in multiple formats (print, live lecture and/or web-based), each format requires a separate application.
  • Incomplete or illegible application forms will not be processed and will be returned to the Program Coordinator for completion.
  • The Program Coordinator is responsible for submitting the initial and renewal application forms. NOTE: Sonography Canada does not send renewal notifications.
  • Application and documentation must be received prior to the scheduled CPD activity.
  • Applications submitted after the CPD activity has taken place will not be accepted. NOTE: Credits will not be granted retroactively. 
  • Allow 6 weeks for application processing. The Program Coordinator will be sent written notification of credit approval.
  • All approved CPD activities are awarded CPD credit for one year from the date of approval.  An approved activity can be offered at multiple times during the approval period. (Exception: a one-time seminar is approved only for the specific date(s) of the seminar).
  • An application, required documentation and processing fee must be submitted to Sonography Canada six weeks prior to the expiration date of the current CPD approval. All CPD activities must be renewed on an annual basis. Failure to provide post-activity documentation may result in the revoking of credits for your program or denial of re-application.

NOTE: CPD numbers are valid for a period of one year. Do not issue certificates with an expired CPD number.

NOTE:  Handwritten information is not acceptable for CPD certificates.

It is the responsibility of the Program Coordinator to:

  • Submit to the CPD committee a complete participant list including:On going programs must submit to the CPD Committee a list of speakers and their credentials, topics and the dates given. (monthly or quarterly).
    • Participant names and Sonography Canada registry number.
    • Number of credits issued to each person. For on-going programs this may be done monthly, quarterly, or semi-annually.                       
  • Ongoing programs must submit to the CPD Committee a list of speakers and their credentials, topics and the dates given. (monthly, quarterly, or semi annually).
  • Ensure evaluations are provided for the participants to complete at the conclusion of the CPD activity.
  • The Program Coordinator is not required to submit the evaluation summary to Sonography Canada. However, Sonography Canada reserves the right to request the evaluation summary in the event questions arise about any activity approved for credit.
  • Ensure each participant receives a CPD certificate within one month of the event. These certificates must contain the following information:Ensure that your participants understand that it is their responsibility to upload these certificates onto the CPD online tracking system.
    • Provider's name
    • Participants name
    • Title of the CPD activity (please make sure that the title matches the title indicated on the application)
    • Date of attendance
    • Number of CPD credits awarded (1 CREDIT FOR EACH HOUR EDUCATION)
    • Sonography Canada CPD file number
    • Signature of the Program Coordinator
  • Ensure that your participants understand that it is their responsibility to upload these certificates onto the CPD online tracking system.
  • Maintain all records of attendance and number of credits granted for three (3) years.
  • Refer to the Sonography Canada CPD file number on all correspondence.
  • For ongoing programs, a new application must be made each year. To ensure continuous coverage this should be done before the current CPD file expires, six weeks prior to the expiration of the current CPD file. 

 Note: Sonography Canada does not send renewal notifications.

On-Going Study Groups and Rounds:

 

Guidelines:

  • This is an educational activity sponsored by a specific institution (ie. hospital, medical/imaging center, clinic or physician’s office) to provide continuing education for employees who are directly involved in the practice of sonography.
  • The study group must have a Program Coordinator.
  • Meetings over 2 hours in length are considered seminars and require a separate application.
  • Approval will be granted for a period of one year. The Program Coordinator is responsible for submitting the initial and renewal application.  

 NOTE: Sonography Canada does not send renewal notifications.

Pre-meeting documentation:

  1. CPD application form, appropriate fees and proof of Sonography Canada chapter/corporate membership (photocopy of membership card if applying for the reduced membership rate).
  2. Submit list of topics and speakers (if known in advance)

Post-meeting documentation:

  1. The program coordinator provides a certificate of completion to each participant.
  2. Ensure that your participants understand that it is their responsibility to upload their certificates onto the online CPD tracking system.
  3. At the end of your program you must submit to Sonography Canada the following information:
    • The date, topic and speaker of each session and the number of credits assigned to that session
    • The total number of credits that you have issued to your activity for the year ie. 15 meetings, each 1 hour = 15 credits
    • A roster including the participants’ names, Sonography Canada number for each session and total of credits issued to each participant
  4. Evaluation forms and summaries do not need be sent Sonography Canada.  However, these must be retained on file for a period of 3 years.  

National, Regional and Local Seminars / Conferences / Courses / Workshops:

These are educational activities consisting of one or more lectures or presentations. The terms seminar, conference, meeting and program are used interchangeably.

Classifications
One-time - a seminar conducted within a specified time frame and offered one time
Multiple offerings - seminar presented multiple times within Sonography Canada CPD approval period.

Guidelines:

  • Credits are given for actual educational time. Registration, introductions, meals and breaks are not calculated in this time.
  • If the content or length of the seminar changes, submit these changes to Sonography Canada.
  • The sequence in which the contents of a seminar are presented can be altered/revised as long as the content and the length of the seminar remains the same.
  • If there is a change/replacement in presenters, submit the revised documentation to the CPD committee.
  • Once received, please quote your Sonography Canada CPD number on all correspondence.
  • The application fee is based on the number of approved Sonography Canada CPD credits issued per year. Issuing more credits than applied for or than approved by Sonography Canada will result in revocation of CPD approval. Also, failure to provide post activity documentation may result in the revocation of credits for your program or denial of re-application.

Application Procedures
Pre-meeting documentation:

  1. CPD application form, appropriate fees and proof of Sonography Canada chapter/corporate membership (photocopy of membership card if applying for the reduced membership rate).
  2. Agenda/ schedule, including lecture times, if the seminar is more than 2 hours in length. The schedule must be consistent with the number of CPD credits requested.

NOTE: Allow 6 weeks for processing of your application.

Post-meeting documentation:

  1. The program coordinator provides a certificate of completion to each participant.
  2. Ensure that your participants understand that it is their responsibility to upload their certificates onto the online CPD tracking system.
  3. Within 60 days after the seminar, the program director submits the Sonography Canada participant roster; the roster must include the participants' full name, Sonography Canada registry number, and the number of CPD credits awarded.

Policy and Procedures for Awarding CPD Credits for Clinical Instructors/Preceptors and CCSA Assessors

Policy: Sonographers actively involved in the clinical teaching/precepting of sonographers, medical and allied health students or professionals from CMA accredited training programs and Sonography Canada CCSA assessors are eligible to receive CPD credits.  NOTE: Preceptor training/workshops can also be submitted for CPD credit approval.

Objectives:

  1. To encourage more sonographers to become involved in the clinical teaching of students enrolled in recognized programs.
  2. To promote greater involvement of those sonographers who are involved in clinical training.
  3. To award the efforts of those sonographers in a practical way by granting them CPD credits. It is realized that these sonographers often receive no financial or other incentives.

Procedure:

Program Coordinator of recognized programs in Canada are eligible to apply for this benefit, on behalf of their clinical instructors/preceptors, on an annual basis.

  1. The standard CPD application and corresponding fee must be submitted to Sonography Canada.
  2. The Program Coordinator would apply under the category of clinical preceptor and/or CCSA assessor on the CPD application.
  3. Once approved by Sonography Canada, the Program Coordinator would distribute the Preceptor CPD Credit Record provided or similar form to the eligible sonographers associated with their program.
  4. Once completed, the document is returned to the Program Coordinator who will issue CPD credits/certificates to the eligible sonographers. 
  5. The number of credits that can be awarded are:
  • 1 CPD credit per day of instruction (~8 hours), to a maximum of 8 CPD credits per year for a clinical instructor/preceptor
  • 3 CPD credits for being a CCSA assessor per year

Guidelines:

Clinical teaching will encompass those cases where the sonographer observes and gives technical tips and suggestions while the student scans all or part of the examination. The sonographer then reviews the images after the case and discusses anatomy, pathology, pathophysiology, technique, differential diagnosis, expected clinical signs and/or lab findings. Information will be reviewed by both sonographer and student and discussed re: pathology, anatomy etc. In a situation where the student only observes a sonographer scan or scans without the direct supervision of a sonographer, does not meet the above criteria and will not be considered applicable for CPD credits under this activity criterion.
